Send Form with data at the agent end from the customer end

Chat conversation between customer and agent working fine. Agent send a Form with fields (Name, Surname) to the Customer.(See screen shot) but the issue is when customer fill this form and send back to the agent then issue is that customer is unable to send form back to the agent .
so help me out that how can we do this by code or by any existing api ?

Have you implemented this form with our Moments?
If so you could use Moments SDK
to pass data as customProperties.
